Coronado Physicians supporting athletics
In the 1990’s Dr. Chuck Eaton and a handful of local physicians donated their services to provide physical exams for Coronado High School athletes. Dr. Eaton’s legacy lives on today with generous support from an expanding list of Coronado physicians & health care professionals. Physicals for Sports ‘10 will be offering both CHS & CMS athletes an opportunity to get the required physical exam necessary to participate in school sports

5:30 pm start for last names A – L
6:00 pm start for last names M – Z
Last call @ 6:30 pm
Closed @ 7:00 pm
All proceeds will fund purchasing of supplies for CHS athletic training room
PLEASE FOLLOW THESE INSTRUCTIONS PRIOR TO THIS EVENT:
1. Register your athlete using the on-line system at:
http://www.islandersportsfoundation.com
2. After completing step 1, you will receive links to necessary forms including:
-Athletic History form (parent to fill out PRIOR to exam)
-Physical Exam form
3. Bring BOTH pages & Cash/Check (made out to Islander Sports Foundation)
Islander Sports Foundation is a non-profit organization supporting CHS & CMS sports
$15/Student
$25 for 2 (same family)
$10/Student for 3 or more (same family)
